Main Street Clinical Associates

Main Street Clinical Associates, the covered entity (CE), reported that an adjacent building suffered a severe gas explosion, causing employees to immediately evacuate the office without the opportunity to secure the medical records in its file room. This breach affected the protected health information (PHI) of 3,031 individuals. In addition, a cell phone, laptops, and a printer were stolen from its office. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also offered affected individuals one year of complimentary credit monitoring services. Following the breach, the CE implemented new physical, technical, administrative, and security safeguards to better protect its sensitive data. Staff members were also retrained. OCR obtained assurances that the CE implemented the corrective actions noted.
