Main Line Endoscopy Centers

The covered entity (CE), Main Line Endoscopy Centers, reported than an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 14,305 individuals. The PHI involved included names, addresses, dates of birth, medical records numbers, health insurance information, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI. OCR provided technical assistance to the covered entity regarding its business associate agreements. OCR obtained assurances that the CE implemented the corrective actions noted.
