The business associate (BA), Luxottica of America, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 829,454 individuals. The PHI involved included addresses, names, dates of birth, claims and financial information, Social Security numbers, diagnoses, conditions, medications prescribed, and other treatment information. The BA notified HHS, affected individuals, the media, and provided complimentary credit monitoring services to affected individuals. In response to the breach, the BA implemented additional technical safeguards. OCR provided the BA with technical assistance regarding its HIPAA Security Rule obligations.
