Long Island Jewish Forest Hills Hospital

The covered entity (CE), Long Island Jewish Forest Hills Hospital, reported that an employee impermissibly accessed the protected health information (PHI) of 10,333 individuals. The PHI involved included names, dates of birth, addresses, phone numbers, health insurance information, medical record numbers, clinical information, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards. Workforce members were retrained on methods to better protect its PHI.
