The business associate (BA), LogicGate, reported that it experienced a cyber-attack affecting the protected health information (PHI) of 47,035 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, financial and claims information, diagnoses, medications, and other treatment information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect PHI.
