Lindner Center of Hope

The covered entity (CE), Lindner Center of Hope, reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 3,537 individuals. The ePHI involved included names, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In addition, the CE provided complimentary credit monitoring services to affected individuals. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on security awareness with a specific focus on recognizing phishing emails. OCR obtained assurances that the CE implemented the corrective actions noted.
