Landmark Holdings of Florida

The covered entity (CE), Landmark Holdings of Florida, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 501 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, diagnoses, lab results, medications, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
