Lakewood Health System

The covered entity (CE), Lakewood Health System, reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of approximately 1,415 individuals. The ePHI involved included names, Social Security numbers, addresses, dates of birth, drivers’ license information, diagnoses, medications prescribed, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. The CE also provided complimentary credit monitoring services. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative, technical, and security safeguards. OCR obtained assurances that the CE implemented the corrective actions noted.
