The covered entity (CE), La Casa de Salud, reported that an employee was the victim of an email phishing attack affecting the protected health information (PHI) of 9,969 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, medical record numbers, health insurance information, diagnoses, medication information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on email security.
