The covered entity (CE), Klamath County, reported that a workforce member impermissibly emailed the protected health information (PHI) of 547 individuals to a personal email account. The PHI involved included names, addresses, dates of birth, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE sanctioned the workforce member responsible for the breach and retrained its workforce on safeguarding PHI. OCR provided technical assistance to the CE regarding the HIPAA Rules.
