The covered entity (CE), Kentuckiana Regional Planning and Development Agency, reported that several employees were the victims of an email phishing scheme involving the electronic protected health information (ePHI) of 3,663 individuals. The ePHI involved included names, dates of birth, Social Security numbers, physical addresses, drivers’ license numbers, diagnoses, health insurance information, and claims and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. In addition, all workforce members were retrained on the HIPAA Privacy and Security Rules.
