Kennedy University Hospital t/a Jefferson Health, the covered entity (CE), reported that an employee lost an external hard drive for a medical imaging device. The device contained the protected health information (PHI) of 4,140 individuals. The PHI involved included names, dates of birth, phone numbers, addresses, medical record numbers, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data.
