Kaiser Foundation Health Plan of the Mid-Atlantic States

The covered entity (CE), Kaiser Foundation Health Plan of the Mid-Atlantic States, Inc., reported that an employee impermissibly accessed the electronic protected health information (ePHI) of 2,756 individuals. The ePHI involved included names, addresses, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE sanctioned the employee involved and implemented additional administrative, technical, and security safeguards to better protect its ePHI.
