Kaiser Foundation Health Plan

Kaiser Permanente (KP), the covered entity (CE), reported that an employee sent the protected health information (PHI) of 515 individuals to the wrong recipients. The PHI involved included names, and health insurance information. The CE notified HHS, affected individuals, and the media. In response to this breach incident, KP implemented additional administrative safeguards to prevent future incidents of this nature.
