Jefferson Healthcare

The covered entity (CE), Jefferson Healthcare, reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of approximately 2,543 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, financial information, claims information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on the importance of recognizing and responding to fraudulent email communications and other cybersecurity threats.
