iTrust Wellness Group

The covered entity (CE), iTrust Wellness Group, reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 981 individuals. The PHI involved included names, phone numbers, email addresses, dates of service, claims information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. The CE also retrained its workforce members on email security precautions.
