Iredell-Statesville Schools Board of Education, the covered entity (CE), reported that an employee of its business associate (BA) was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 959 individuals. The ePHI involved included names, Social Security numbers, financial information, and treatment information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented new administrative and technical safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
