The covered entity (CE), Iowa Total Care, reported that an employee inadvertently emailed the protected health information (PHI) of 536 individuals to the wrong recipient. The PHI involved included names, dates of birth, health insurance and claims information, diagnoses, and other treatment information. In response to the breach, the CE retrained all employees on email security precautions.
