Interim HealthCare of the Capital Region

The covered entity (CE), Interim HealthCare of the Capital Region, reported that several employees were the victims of an email phishing scheme that affected the protected health information (PHI) of 1,127 individuals. The PHI involved included names, dates of birth, addresses, financial and health insurance information, diagnoses, medical record numbers, Social Security numbers, and drivers' license numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
