Intellihartx, LLC, the business associate (BA), reported that it experienced a cyber-attack that affected the protected health information (PHI) of 489,830 individuals. The PHI involved included names, dates of birth, social security numbers, diagnoses and conditions, medications, and other treatment information. The BA notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the BA implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
