The covered entity (CE), InSider Vision, LLC, reported that an employee emailed the protected health information (PHI) of 1,426 individuals to the wrong recipient. The PHI involved included names, phone numbers, and addresses. The CE notified HHS and impacted individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
