Indiana State Medical Association, the covered entity (CE), reported that two hard drives containing the electronic protected health information (ePHI) of approximately 38,351 individuals were stolen from an employee’s vehicle. The ePHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, and clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Affected individuals were offered free credit monitoring services. In its mitigation efforts, the CE sanctioned the employee involved and implemented new physical, administrative, and technical safeguards to prevent future incidents of this nature. The CE also retrained its staff on the importance of safeguarding ePHI. OCR obtained assurances that the CE implemented the corrective actions noted.
