Hutton & Hale, D.D.S

The covered entity (CE), Hutton & Hale, DDS, reported that it was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 8,394 individuals. The ePHI involved included names, addresses, email addresses, Social Security numbers, and clinical information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In its mitigation efforts, the CE implemented additional administrative and security safeguards to better protect its ePHI.
