The covered entity (CE), Human Development Center, reported that a workforce member was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 1,200 individuals. The ePHI involved included names, dates of birth, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical and security safeguards and retrained its staff on recognizing and responding to fraudulent email communications. OCR provided technical assistance to the CE and obtained assurances that the CE implemented the corrective actions noted.
