Hospice and Palliative Care Charlotte Region

The covered entity (CE), Hospice and Palliate Care Charlotte Region dba VIA Health Partners, reported that an employee of its business associate (BA) was the victim of an email phishing scheme that affected the protected health information (PHI) of 777 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses/conditions, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional technical and security safeguards to better protect its PHI.
