The covered entity (CE), Home State Health Plan, Inc., reported that its business associate (BA) sent letters containing the protected health information (PHI) of 1,020 individuals, to the wrong recipients. The PHI involved included names and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative and technical safeguards to better secure its PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
