The covered entity (CE), HMG Healthcare, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 80,000 individuals. The PHI involved included names, dates of birth, Social Security and drivers’ license numbers, addresses, claims and financial information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
