Heavenly Hands Family Medical Clinic

The covered entity (CE), Heavenly Hands Family Medical Clinic, reported that its business associate (BA) accessed it's electronic health record system that contained the electronic protected health information (ePHI) of 1,837 individuals, without proper authorization. The ePHI involved included names, dates of birth, Social Security numbers, financial information, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. As a result of the breach, the CE strengthened its administrative and technical safeguards and terminated its agreement with the BA.
