The covered entity (CE) reported a breach concerning the theft of a laptop computer from its medical office. The laptop was used for eye scans and contained the names, dates of birth, and medical record numbers of 1,300 patients. The CE provided breach notification to HHS, affected individuals, and the media. As a result of OCR’s investigation, and to prevent a similar breach from happening in the future, the CE undertook a comprehensive risk analysis, encrypted its mobile devices, and ensured that physical safeguards were in place. It also retrained employees and revised its security policies and procedures.
