Healthfirst, the covered entity (CE), reported that employees sent letters containing the protected health information (PHI) of 1,811 individuals to the wrong addresses. The PHI involved included names, birthdates, diagnoses, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented new administrative safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
