Health Plan of San Joaquin

The covered entity (CE), Health Plan of San Joaquin, reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 418,842 individuals. The ePHI involved included names, Social Security numbers, drivers’ license numbers, health insurance information, claims and financial information, dates of birth, lab results, prescription information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. The CE also offered complimentary credit monitoring services. In response to the breach, the CE updated its email access protocols and implemented additional technical safeguards.
