The covered entity (CE), Health Insurance Plan of Greater New York, reported that an employee inadvertently mailed documents that contained the protected health information (PHI) of 792 individuals to the wrong recipients. The PHI involved included names, member identification numbers, and claims and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE strengthened its administrative safeguards to better protect its PHI.
