The covered entity (CE), Health First reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 14,171 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE sanctioned the responsible employee and implemented additional technical and security safeguards.
