The covered entity (CE), Hazleton Anesthesia Services, reported that its business associate (BA) suffered a ransomware attack which affected the protected health information (PHI) of 13,560 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards and provided complimentary credit monitoring services to affected individuals.
