Hawaiian Eye Center

The covered entity (CE), Hawaiian Eye Center, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 14,524 individuals. The PHI involved included names, addresses, email addresses, dates of birth, drivers’ license numbers, Social Security numbers, health insurance information, and other treatment data. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and strengthened its technical and security safeguards, revised policies and procedures, and implemented additional training for its staff. During the investigation, OCR provided the CE with technical assistance regarding the HIPAA Rules.
