Hao Rong, DDS, Inc., dba Genuine Care Dental, reported that an intruder broke into its office and stole a decommissioned server that contained the electronic protected health information (ePHI) of 2,190 individuals. The ePHI involved included names, birthdates, addresses, Social Security numbers, drivers’ license numbers, claims and financial information, diagnoses, medications prescribed, clinical information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach incident, the CE implemented additional physical, technical, and administrative safeguards to better protect its ePHI. All workforce members were retrained.
