The PHI involved included names, dates of birth, addresses, drivers’ license and social security numbers, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE improved physical safeguards at its facility and implemented additional administrative safeguards.
