The covered entity (CE), Gyrodata, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 944 individuals. The PHI involved included names, addresses, dates of birth, drivers' license numbers, Social Security numbers, passport numbers, and tax and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards and provided complimentary credit monitoring services to affected individuals.
