Griffith Energy Services Inc. Welfare Benefit Plan

OThe covered entity (CE), Griffith Energy Services Welfare Benefit Plan, reported that it was the victim of a cyber-attack that affected the protected health information (PHI) of 2,693 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE updated its policies and procedures, implemented additional physical and technical safeguards, and retrained its staff. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule.
