Granite Falls Family Medical Care Center

The covered entity (CE), Granite Falls Family Medical Care Center, reported that its business associate (BA) was the victim of a ransomware attack that affected the protected health information (PHI) of 5,404 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services to those affected. OCR provided the CE with technical assistance pertaining to the HIPAA Privacy Rule.
