The covered entity (CE), The Geo Group, reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 3,991 individuals. The ePHI involved included names, dates of birth, Social Security numbers, addresses, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and retrained its employees on the proper methods of detecting and responding to fraudulent email communications. OCR provided the CE with technical assistance regarding the Breach Notification Rule.
