The covered entity (CE), Geisinger Wyoming Valley Medical Center, reported that a workforce member impermissibly accessed the electronic protected health information (ePHI) of 805 individuals. The ePHI involved included names, dates of birth, addresses, phone numbers, e-mail addresses, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the responsible employees and implemented additional administrative safeguards to better protect its ePHI.
