The covered entity (CE), Gastroenterology Consultants, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 161,698 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, lab results, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE strengthened its administrative and technical safeguards. OCR provided technical assistance regarding the HIPAA Rules.
