Gainwell Technologies, the business associate (BA), reported that it experienced a security incident in which an unauthorized individual was able to obtain login credentials from an employee. This breach affected the protected health information (PHI) of 810 individuals. The PHI involved included names, member identification numbers, and diagnoses. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI.
