Frilot

The business associate (BA), Frilot L.L.C., reported that it experienced a hacking incident that affected the electronic protected health information (ePHI) of 1,448 individuals. The PHI involved included names, dates of birth, Social Security numbers, driver’s license numbers, addresses/zip codes, medications, diagnoses/conditions, lab results, and other treatment information, and claims information. The BA notified HHS and affected covered entities. In response to the breach, the BA implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI.
