Fred's Stores of Tennessee

Fred’s Stores of Tennessee, the covered entity (CE), reported that an unencrypted laptop had been stolen from the vehicle of an employee exposing the electronic protected health information (ePHI) of 9,624 individuals. The ePHI involved included names, telephone numbers, and medications prescribed. During the course of our investigation, the CE filed for bankruptcy and ceased operations.
