Foundation Surgical Hospital of San Antonio

The covered entity (CE), Foundation Surgical Hospital of San Antonio, reported that the electronic protected health information (ePHI) of 1,237 individuals was viewable on its website. The ePHI involved included names, diagnoses, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. In addition, the CE sanctioned the workforce members responsible for the breach and retrained its workforce on its security obligations. OCR obtained assurances that the CE implemented the corrective actions noted.
