The covered entity (CE), Fondren Orthopedic Group, reported that it experienced a cyber-attack that affected the electronic protected health information (ePHI) of approximately 30,049 individuals. The ePHI involved included names, addresses, telephone numbers, dates of birth, health insurance information, and medication information. The CE notified HHS, affected individuals, the media and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI.
