Foleck Center

The Foleck Center, the covered entity (CE), reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 6,965 individuals. The PHI involved included names, birthdates, Social Security and drivers’ license numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data and re-trained staff on security awareness.
