The covered entity (CE), Florida Medical Clinic, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 94,132 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring to affected individuals and implemented additional administrative, technical, and security safeguards.
